Xbox One Bugs that keep popping up

So some problems or bugs are that one my game some times freezes and slows down but I can still move. Two sometimes when I exit my game and go to my characters then are replaced with black lettering and I have to restart my game because it will mostly have really messed up graphics. Three some times I can only see the particals of my weapons when to many projectiles are use. And that's all I can think of right now.
