It's usually a high damage fire elite attack from off screen for me.

Well it could also be a void enemy, collapse is just so strong for them.

Hm. I usually get destroyed by any elite version of any of the later enemies.
I forget the names at this time, but the bell enemies and the beetle guards come to mind. Along with those raven things.

To say nothing of that inexplicable barrage of those moon enemies the second you get on Petrichor V's moon.

I usually don't have much of an issue with void crawlers, oddly enough.

If you stand still Brass Contraptions often have a hard time hitting you.
Although thats mainly when there's like 1 or 2.
