Are you using another mod that grants an additional accessory slot (such as Enigma or MoreAccessories+)? If you already have a slot from those mods then the onion will not function.

Do you have the RevengeanceAndDeathThoriumBossBuff line in the mod config enabled? It's currently bugged in 1.4.2.106 and causes a crash. A fix for it will be coming in a small patch before the next big update, but for now you should try disabling it. The patch should be coming out pretty soon...
