Items Brain of Confusion and Worm Scarf

Since the Worm Scarf reduces invoming damage, could the Brain increase the player's damage instead? Like, a simple damage buff.

Also, since many say that the scarf is op, could both the Brain and the Scarf have a positive and a negative side? Example:
Scarf: -17% damage from enemies, but -5% damage to enemies as well
Brain: +10% damage, but -10% defence

Thoughts?
 
The Brain and the Scarf are alternates but they are not equal, the brain does one thing, the scarf is made for an entirely different purpose. I do not think it would work and the negative side-effects are just bad changes for the items that require quite a great challenge to actually get.
 
This is the simple way of fixing the Brain of Confusion, making it a combat item with a secondary effect which sometimes shows up.

Worm scarf doesn't need a nerf nor does Brain of Confusion need a downside like that since it's already bad.
