The leviathan is a structure that is found near the boundary between the underworld and the caverns. It is made of gristle blocks and tooth blocks. Gristle blocks and tooth blocks require a pickaxe power of 65 to mine. Gristle blocks are white and shiny-looking, and tooth blocks are a very pale yellow. It is structured like a skeleton, with a skull, spine, ribcage, and tail made of gristle blocks, and a wing, pelvic fin, caudal fin, and set of teeth made from tooth blocks. The wing is attached near the front of the ribcage, and the pelvic fin is not directly attached to the rest of the skeleton.

Between the pelvis fin and ribcage is a small space with a water chest inside, which, in addition to the usual contents, contains toxic paste. Once you obtain toxic paste, more of it can be bought from the dryad. Toxic paste is used to make the vermicidal potion, which is a throwable potion. It is made with bottled water, toxic paste, a stinkfish, and deathweed. Behind the skull are a small vein of gill blocks. Gill blocks are bright-red and fleshy-looking. Throwing a vermicidal potion at the gill blocks in hardmode will summon the kilbit.

The kilbit is a worm boss. It has a maximum life of 40000hp. Each segment can deal 50hp of damage, but has no defence. It can be poisoned for 60s with the vermicidal potion, and also will spawn poisoned for the same amount of time. It starts out 8 segments long, but will grow a segment every 20s, until it reaches 13 segments, when the final 5 body segments will break away as epitokes, with the tail segment dying, and a new tail segment appearing on the end of the kilbit. These epitokes have no defence and a maximum life of 200hp. They will deal 40hp of damage when they touch the player, but will dissappear. They will also cause the infected debuff. The infected debuff does not do anything, but when it ends, an immature kilbit will spawn from the player (or whatever else has the debuff). Immature kilbits are smaller versions of the kilbit boss, with a maximum life of 500hp, and each segment dealing 45hp of damage. They do not grow and cannot spawn epitokes. In appearance, Kilbit is bright-red, with the head segment having hooks at the front, the body segments having 4 legs each, and the tail segment has a sucker at the end and two clamps on the side. The epitokes look like detached body segments. The immature kilbits look like the adults, except smaller and with a stumpy tail and no head-hooks.

The kilbit drops 10 gold coins, and after it dies the gill blocks can be mined. These gill blocks can then be combined with shroomite bars at an autohammer to create triplex bars. Triplex bars can be made into triplex armour. Triplex armour is dark-red, and has 4 headgear variants. Triplex boots give 18 defence, the triplex chestplate gives 25 defence, and the headgears each give 20 defence. The headgears are the triplex cap, which is a blue, bowl-shaped cap that increases ranged damage by 30%, the triplex mask, which is a bright-red mask resembling a fish-face that increases melee damage by 30%, the triplex hat, which is a green, leafy hat that increases magic damage by 30%, and the triplex helmet, which is a white orb that covers the head and increases summon damage by 30%
