There are many incorrect translations in the Polish version. I understand that such errors may result from translations of the script, without knowing the context from the game itself, so I decided to prepare a list of incorrect translations, more or less explain what some of them result from and propose a more appropriate translation.
But, first of all, I would like to point out that the Polish translation contains special letters that cannot be used when using the journey modes menu, which is very inconvenient.
Now let's get to the list:
1. Boss Masks - here we have not so much an incorrect translation as a translation inconsistency. Here is an example of a good translation:
• Moon Lord Mask - Maska Księżycowego Władcy
For first, some of the masks have the bosses' names written in lowercase letters (in this case, just change the lowercase first letters of the bosses' names to uppercase):
• Deerclops Mask - Maska jelenioka
• Dark Mage Mask - Maska mrocznego maga
• Ogre Mask - Maska ogra
• Golem Mask - Maska golema
• Lunatic Cultist Mask - Maska obłąkanego kultysty
Two masks deserve for special attention.
• Wall of Flesh Mask is the first, which instead of "Maska Ściany Mięcha" it is translated to "Maska - Ściana Mięcha".
• Twin Mask is the second, which istead of "Maska Bliźniąt" it is translated to "Bliźniacza maska", which in this case simply refers to the two-faced mask, rather than The Twins' boss mask.
2. Terrarium/Cage - in Polish, an animal cage is made of bars in any case. In turn, Terrarium sounds the same in our language as in English. These two words are not interchangeable in Polish, so all "Klatka dla zwierząt" should be renamed "Terrarium z zwierzęciem". Moreover, Polish uses the plural form of animals in a cage, although there is always only one.
Some examples of how it should be:
• Bunny Cage - "Terrarium z królikiem" instead of "Klatka dla króliczków"
• Gold Squirrel Cage - "Terrarium ze złotą wiewiórką" insted of "Klatka dla złotych wiewiórek"
• Truffle Worm Cage - "Terrarium z robakiem truflowym" instead of "Klatka dla robak truflowych" (besides, there is an error in combining the singular and plural in the two parts of the Polish name of Worm Truffle)
• Toucan Cage - "Terrarium z Tukanem" instead of "Klatak Tukana" (there is a very strange spelling mistake in the current Polish version)
3. Boss Trophies - in the Polish version of the trophy names, it was decided to write them with a dash, there is nothing wrong with that, but four trophies again use lowercase first letters in the boss names, and one of them also dropped the dash. These are:
• Dark Mage Trophy - uses "Trofeum - mroczny mag" instead of "Trofeum - Mroczny Mag"
• Ogre Trophy - uses "Trofeum - ogr" instead of "Trofeum - Ogr"
• Lunatic Cultist Trophy - uses "Trofeum - obłąkany kultysta" instead of "Trofeum - Obłąkany Kultysta"
• Deerclops Trophy - uses "Trofeum jelenioka" instead of "Trofeum - Jeleniok"
4. Golf Trophies - here we have a situation where the translators treated Golf Trophies the same as Boss Trophies, although they are completely different.
• Bronze Golf Trophy - uses "Trofeum - brązowy golf" instead of "Brązowe trofeum golfowe"
• Silver Golf Trophy - uses "Trofeum - srebrny golf" instead of "Srebrne trofeum golfowe"
• Gold Golf Trophy - uses "Trofeum - złoty golf" instead of "Złote trofeum golfowe"
Currently it sounds like there are three additional bosses in the game, Bronze Golf, Silver Golf and Gold Golf.
5. Bar (wooden furniture) - in this case, it is easy to guess where the error comes from, the translators treated the bar furniture in the same way as material bars. Meanwhile, "Bar" as a piece of furniture sounds the same in Polish as in English, i.e. "Bar".
6. Pet (interaction with NPC animals) - in Polish it was translated as "Chowaniec", which in English does not mean "Pet" but "Familiar". The correct translation of the action is "Głaskanie".
7. Pet Exchange Program - also in this case, the word "Pet" was translated as "Chowaniec" (Familiar), but the correct translation of "Pet" as pets is "Zwierzę". Therefore, instead of "Program wymiany chowańcow" it should be "Program wymiany zwierząt".
8. Dirt Rod - currently "Brudna laska", this translation treats dirt not as ground, but as dirt (e.g. dust on furniture). The proper translation of "dirt" as ground is "ziemia", so instead of "Brudna laska", it should be "Ziemna różdżka".
9. The Dirtiest Block - similar error, the current translation is "Najbrudniejszy blok" refers to dirt like dust on the furniture, not dirt like the ground. The correct translation is "Najbardziej ziemisty blok".
10. Man Eater - here the problem arises from the lack of context, the current translation is "Kanibal" which is identical to the English "Cannibal", which of course makes no sense because cannibals eat their own species. The correct translation is "Ludożerca".
11. World Feeder - in this case, the problem results from the difficult translation of the word "Feeder" into Polish. The current translation is "Żywiciela świata", where "Żywiciela" itself does not mean someone who eats, but is the female form of someone who gives food to others. The correct translation is "Zjadacz świata".
12. Happiness (dialogue with NPC) - here the error is just a typo that changes the declimation case of a given word. Instead of "Szczęścia" it should be "Szczęście".
13. Catfish - this incorrect translation is due to the lack of context for the wordplay. The current translation is "Sum", which does not refer to a cat-shaped fish, but to real catfish. The correct translation is "Kotoryba".
14. Celebration and Celebration Mk2 - the first problem is easy to notice, in the Polish version these weapons are called "Sylwester" and "Świętowanie 2". Besides, "Sylwester" means "New Year's Eve", and "Świętowanie" does not correspond to the name of the weapon, but to the event. The best names would be "Uroczystość" and "Uroczystość Mk2".
15. Necromantic Scroll - there was a simple typo here, in the Polish version we have "Nekromatnyczny Zwój" instead of "Nekromantyczny Zwój", someone mixed up "n" and "t".
16. Badger's Hat - incorrect translation due to lack of context, translators translated the streamer's name as if it were an ordinary animal. That's why we currently have "Kapelusz Borsuka" instead of "Kapelusz Badgera".
17. Shroomerang - in the Polish version the name is "Krewetkowy Bumerang" which literally means "Shrimp Boomerang" and has nothing to do with mushrooms. One of the translators probably confused the word "shroom" with "shrimp", which, however, does not make any logical sense. The correct name is "Grzybowy bumerang" or "Grzyborang".
18. Creeper Set - incorrect translation due to lack of context. The translators probably assumed that it was a set of one of the creepers found in Terrari, and not the one related to Minecraft. Creeper itself is one of the mobs that Mojang doesn't allow to translate, so it's Creeper in every country. So instead of the current "Zestaw Pełzaka" it should be "Zestaw Creepera".
19. Gold Goldfish - here we have an example when translators, wanting to make a good translation, spoiled the word game. Explain this with an example:
• Gold Bunny - Złoty króliczek
• Gold Squirrel - Złota wiewiórka
• Goldfish - Złota rybka
• Gold goldfish - Złocista złota rybka
The translators wanted to separate the two words "gold", but this spoiled the wordplay. Repetitions are allowed in Polish, so it should be "Złota złota rybka".
20. Fish Bowls - it is complicated, Polish allows for some simplifications, so translators translating "Fish Bowl" removed the word "Fish" from it, creating the translation "Kuliste akwarium" (spherical aquarium) instead of "Akwarium z rybką" (aquarium with fish). Previously, this was not a problem because there was only one aquarium. But now we have:
• Fish Bowl - Kuliste akwarium
• Gold Fish Bowl - Złote kuliste akwarium
• Pupfis Bowl - Akwarium z zagrzebką
• Lava Serpent Bowl - Misa lawy z wężem
Only Pupfish Bowl is now well translated. Fish Bowls translation points currently points to an empty awkwria because it does not contain a creature name. In turn, the translation of Lava Serpent Bowl means "Bowl of lava with a snake", not an aquarium with a lava snake. The correct translations are:
• Fish Bowl - Akwarium ze złotą rybką
• Gold Fish Bowl - Akwarium ze złotą złotą rybką
• Pupfis Bowl - Akwarium z zagrzebką
• Lava Serpent Bowl - Akwarium z lawowym wężem
21. Jellyfish Jar - in the English language version, the aquarium was deliberately called a jar, because both words start with "J" and have a funny sound. However, this procedure did not stop the Polish version, where currently we have "Słoik z meduzą" which does not sound, is not funny and has no logic. Therefore, I propose to change "jar" to "aquarium" in the Polish language version:
Blue Jellyfish Jar - Słoik z niebieską meduzą -> Akwarium z niebieską meduzą
Green Jellyfish Jar - Słoik z zieloną meduzą -> Akwarium z zieloną meduzą
Pink Jellyfish Jar - Słoik z różową meduzą -> Akwarium z różową meduzą
22. Life Preserver - incorrect translation due to lack of context. Currently, in the Polish version it is "Obrońca życia" (Defender of Life), someone who defends life, a person. The correct translation is "Koło ratunkowe".
23. Forbidden Treads - the incorrect translation results from the translator confusing the words "Treads" and "Threads", which is why in Polish we have "Zakazane nici" (Forbidden Threads). The correct translation should be "Zakazane obuwie" ("Treads" as "Footwear" is the most fitting Polish translation).
24. Bone Lee - it's really hard to tell what happened here. In Polish "Bone Lee" is "Obrus Lee" (Tablecloth Lee). I don't know where that came from. The literal translation is "Kość Lee" which kills the joke, and the most fitting seems to me to be "Kostek Lee".
25. Brick Layer - here we have a literal translation without content. In Polish "Brick Layer" is currently called "Warsta cegieł" (which literally means bricks arranged in layers), and the tool itself, which is an accessory, is in Polish "Szpachla murarska" or "Kielnia murarska".
26. Extendo Grip - in Polish it was translated as "Większy zasięg" (Greater Range), and the actual translation of this "toy" is "Chwytak". I don't know where the current translation came from.
27. Grandfather Clock - another translation without context. Currently in Polish we have "Zegar dziadka" (a clock belonging to grandfather), and the correct translation is "Zegar stojący".
28. Sweetheart Necklace - here we have an English pun that was tried to be translated without context. Currently in Polish the accessory is called "Naszyjnik miłości" (Love Necklace). Unfortunately it is impossible to translate this pun so I suggest a literal translation "Naszyjnik słodkiego serca" (Sweet Heart Necklace) or shorter "Naszyjnik miodku" (Honey Necklace).
29. Firefly - another strange example of a translation with no known explanation. The current Polish translation is "Iluminator" (Iluminator), and the correct translation is "Robaczek Świętojański". However, since Firefly and Lightning Bug are two names for the same insect, I propose to swap their translations.
In the forests, Firefly will be called "Świetlik", and in Hallow, Lightning Bug will be called "Robaczek świętojański", because "świeto" = "holy", which fits Hallow.
30. The Meatball - is another play on words that cannot be translated. In English, "meatball" is both "a ball made of meat" (which fits cep) and a dish. However, the current Polish name refers only to the dish, and in the diminutive form "Klopsik". The proper translation here is "Kula z mięsa".
31. Web Slinger - the current Polish name is "Pajęczak" (other name for "Spider"), which does not fit as a hook name. The correct name is "Miotacz sieci", which also refers to devices used by Spider-Man.
32. Ivy Whip - since adding actual whips this name is a problem, however, while it sounds in English, it loses its sound in Polish. Currently it is called "Bicz z bluszczu" (a whip made of ivy), and the correct name would be "Bluszczowy hak" (Ivy Hook, because in the Polish version sticking to the name "whip" does not even make sense in the pronunciation).
33. Rubblemaker - the tool for creating ambient objects, the so-called rubble, is called "Miażdżeciel" in Polish (Crusher, something that crushes stones into rubble, but has nothing to do with creating something). Since it is a play on words, I propose giving this tool a fancy name "Obiekt-o-twór" (Objcets-o-maker)
34. Blowpipe and Blowgun - here we have two related weapons with similar names. In Polish they are "Dmuchawka" (correct translation) and... "Dmuchawiec" (dandelion). The problem is that there is no equivalent of the word Blowgun in Polish, but the most correct declination would be "Dmuchawa" (big blowpipe).
35. Ranger Emblem - "Ranger" is a ranged attacker and a forest guard, but the Polish translation is "Symbol Leśnika" (Forest Guard Emblem, meaning without the context of ranged damage). In Polish there is no translation of "Ranger" that refers to a ranged attack, the most fitting translation is "Symbol Strzelca" (Shooter/Gunman/Archer Emblem).
36. Snow Balla and Balla Hat - gere again we have a play on words that could not be translated. In Polish, Snow Balla is "Śnieżny Koleżka" (Snow Buddy). A fitting translation would be "Śnieżny Miotacz" (Snow Thrower). In turn, Balla Hat is currently "Kapelusz Cwaniaka" (Smartass Hat) in Polish, since "Balla" itself has no translation. A correct translation that goes hand in hand with the new translation of Snow Balla would be "Kapelusz Miotacza" (Thrower Hat).
37. Gangsta Hat - here we have only a bad variation in relation to other hats. Currently it is called "Kapelusz gangsterski', and correctly it should be "Kapelusz gangstera".
38. Brain Scrambler (item) - currently in Polish this item is called a "Mąciciel" (Troublemaker). If this item (in the graphic) is supposed to be a brain-confusing remote control, it should be called "Mózgo-mieszacz" (Brain-scrambler).
39. Everscream - originally it is a play on words "evergreen" + "scream". However in Polish it is called "Ciągłokrzyk" (eternal scream, without reference to evergreen). The correct translation would be "Choinkokrzyk" (Christmas tree scream).
40. Demolitionist - there's a translation problem here, most likely due to lack of context. A demolitionist in Terraria is someone who sells bombs, but currently in Polish it's called "Dewastator" (a person who devastates the area, e.g. uproots flowers, spray paints bus stops and takes down road signs). The literal translation is "Rozbiurkowiec", but it's a person who demolishes buildings. The most fitting translation, which refers to bombs and is recognized in games, might be "Bombiarz" (Bomber/person who uses bombs).
Additionally, if "Bombiarz" passes, it would be fun to give him a possible name Bob (Bombiarz Bob), which would also refer to the yellow helmet of the character from the children's cartoon (Bob the Buldeir).
41. Fairy and Pixie - fairies are friendly creatures that help players, can be pets and can be used as furniture. Pixies are mean and attack players. Unfortunately, in Polish, fairy and pixie have one translation "Wróżka" and it is assigned to Pixie. Fairy was translated as "Duszek" (little spirit), which really doesn't fit the character's portrayal. Funnily enough, the achievement for meeting Fairy in the caves uses the word "Wróżka". In Poland, a fairy is commonly associated with a small, good creature, so the translation "Wróżka" should go from Pixie to Fairy and all items related to them. Pixie, on the other hand, as a mischievous variety of fairy, can be renamed "Złośliwy duszek" (mischievous little ghost) or remain under the name "Pixie", which is rare, but occurs in Poland.
42. Ash Content - in version 1.4.4. the Ash Forest was added to Hell. However, in the Polish version of the game, most likely due to the lack of contact text, instead of a forest associated with Ash (Block), the content was translated from a tree from the real world, an ash tree. So in Poland we have not ash content, but wood content. Here are examples:
• Ash Grass - currently "Jesionowa trawa" (Ash Tree Grass), should be "Popiołowa trawa".
• Ash Wood - currently "Drewno jesionowe" (real world Ash Tree wood), should be "Drewno popiołowe".
• Ash Wood Work Bench - currently "Jesionowy stół warsztatowy" (Ash Tree (irl) Work Bench), should be "Stół warsztatowy z drewna popiołowego" (corrected name arrangement based on other furniture made of different types of wood).

24. Bone Lee - it's really hard to tell what happened here. In Polish "Bone Lee" is "Obrus Lee" (Tablecloth Lee). I don't know where that came from. The literal translation is "Kość Lee" which kills the joke, and the most fitting seems to me to be "Kostek Lee".
25. Brick Layer - here we have a literal translation without content. In Polish "Brick Layer" is currently called "Warsta cegieł" (which literally means bricks arranged in layers), and the tool itself, which is an accessory, is in Polish "Szpachla murarska" or "Kielnia murarska".
26. Extendo Grip - in Polish it was translated as "Większy zasięg" (Greater Range), and the actual translation of this "toy" is "Chwytak". I don't know where the current translation came from.
27. Grandfather Clock - another translation without context. Currently in Polish we have "Zegar dziadka" (a clock belonging to grandfather), and the correct translation is "Zegar stojący".
28. Sweetheart Necklace - here we have an English pun that was tried to be translated without context. Currently in Polish the accessory is called "Naszyjnik miłości" (Love Necklace). Unfortunately it is impossible to translate this pun so I suggest a literal translation "Naszyjnik słodkiego serca" (Sweet Heart Necklace) or shorter "Naszyjnik miodku" (Honey Necklace).
29. Firefly - another strange example of a translation with no known explanation. The current Polish translation is "Iluminator" (Iluminator), and the correct translation is "Robaczek Świętojański". However, since Firefly and Lightning Bug are two names for the same insect, I propose to swap their translations.
In the forests, Firefly will be called "Świetlik", and in Hallow, Lightning Bug will be called "Robaczek świętojański", because "świeto" = "holy", which fits Hallow.
30. The Meatball - is another play on words that cannot be translated. In English, "meatball" is both "a ball made of meat" (which fits cep) and a dish. However, the current Polish name refers only to the dish, and in the diminutive form "Klopsik". The proper translation here is "Kula z mięsa".
31. Web Slinger - the current Polish name is "Pajęczak" (other name for "Spider"), which does not fit as a hook name. The correct name is "Miotacz sieci", which also refers to devices used by Spider-Man.
32. Ivy Whip - since adding actual whips this name is a problem, however, while it sounds in English, it loses its sound in Polish. Currently it is called "Bicz z bluszczu" (a whip made of ivy), and the correct name would be "Bluszczowy hak" (Ivy Hook, because in the Polish version sticking to the name "whip" does not even make sense in the pronunciation).
33. Rubblemaker - the tool for creating ambient objects, the so-called rubble, is called "Miażdżeciel" in Polish (Crusher, something that crushes stones into rubble, but has nothing to do with creating something). Since it is a play on words, I propose giving this tool a fancy name "Obiekt-o-twór" (Objcets-o-maker)
34. Blowpipe and Blowgun - here we have two related weapons with similar names. In Polish they are "Dmuchawka" (correct translation) and... "Dmuchawiec" (dandelion). The problem is that there is no equivalent of the word Blowgun in Polish, but the most correct declination would be "Dmuchawa" (big blowpipe).
35. Ranger Emblem - "Ranger" is a ranged attacker and a forest guard, but the Polish translation is "Symbol Leśnika" (Forest Guard Emblem, meaning without the context of ranged damage). In Polish there is no translation of "Ranger" that refers to a ranged attack, the most fitting translation is "Symbol Strzelca" (Shooter/Gunman/Archer Emblem).
36. Snow Balla and Balla Hat - gere again we have a play on words that could not be translated. In Polish, Snow Balla is "Śnieżny Koleżka" (Snow Buddy). A fitting translation would be "Śnieżny Miotacz" (Snow Thrower). In turn, Balla Hat is currently "Kapelusz Cwaniaka" (Smartass Hat) in Polish, since "Balla" itself has no translation. A correct translation that goes hand in hand with the new translation of Snow Balla would be "Kapelusz Miotacza" (Thrower Hat).
37. Gangsta Hat - here we have only a bad variation in relation to other hats. Currently it is called "Kapelusz gangsterski', and correctly it should be "Kapelusz gangstera".
38. Brain Scrambler (item) - currently in Polish this item is called a "Mąciciel" (Troublemaker). If this item (in the graphic) is supposed to be a brain-confusing remote control, it should be called "Mózgo-mieszacz" (Brain-scrambler).
39. Everscream - originally it is a play on words "evergreen" + "scream". However in Polish it is called "Ciągłokrzyk" (eternal scream, without reference to evergreen). The correct translation would be "Choinkokrzyk" (Christmas tree scream).
40. Demolitionist - there's a translation problem here, most likely due to lack of context. A demolitionist in Terraria is someone who sells bombs, but currently in Polish it's called "Dewastator" (a person who devastates the area, e.g. uproots flowers, spray paints bus stops and takes down road signs). The literal translation is "Rozbiurkowiec", but it's a person who demolishes buildings. The most fitting translation, which refers to bombs and is recognized in games, might be "Bombiarz" (Bomber/person who uses bombs).
Additionally, if "Bombiarz" passes, it would be fun to give him a possible name Bob (Bombiarz Bob), which would also refer to the yellow helmet of the character from the children's cartoon (Bob the Buldeir).
41. Fairy and Pixie - fairies are friendly creatures that help players, can be pets and can be used as furniture. Pixies are mean and attack players. Unfortunately, in Polish, fairy and pixie have one translation "Wróżka" and it is assigned to Pixie. Fairy was translated as "Duszek" (little spirit), which really doesn't fit the character's portrayal. Funnily enough, the achievement for meeting Fairy in the caves uses the word "Wróżka". In Poland, a fairy is commonly associated with a small, good creature, so the translation "Wróżka" should go from Pixie to Fairy and all items related to them. Pixie, on the other hand, as a mischievous variety of fairy, can be renamed "Złośliwy duszek" (mischievous little ghost) or remain under the name "Pixie", which is rare, but occurs in Poland.
42. Ash Content - in version 1.4.4. the Ash Forest was added to Hell. However, in the Polish version of the game, most likely due to the lack of contact text, instead of a forest associated with Ash (Block), the content was translated from a tree from the real world, an ash tree. So in Poland we have not ash content, but wood content. Here are examples:
• Ash Grass - currently "Jesionowa trawa" (Ash Tree Grass), should be "Popiołowa trawa".
• Ash Wood - currently "Drewno jesionowe" (real world Ash Tree wood), should be "Drewno popiołowe".
• Ash Wood Work Bench - currently "Jesionowy stół warsztatowy" (Ash Tree (irl) Work Bench), should be "Stół warsztatowy z drewna popiołowego" (corrected name arrangement based on other furniture made of different types of wood).

I spent a few days (with a break during the holidays) preparing this list, I tried to make it as clear as possible and show what the problem is, so I really hope that you will notice it and improve the translations in the upcoming update.

Probably due to lack of context. Imagine you get a file, a blank page where you have a table with the names of all blocks, items, NPC texts, splashes, bestiary entries, enemy names, every lettered word/sentence in the game. In Terraria, that's a lot, probably over 10,000 subitems. Of course, every translator is human, so they won't remember every item in the game, and if they had to check everything in real time, e.g. whether wood is really wood, they would never finish. They translate some items even before they hit the game, so it's obvious that they may lack context or make some mistake.
Plus there are puns that you can't tell are puns until you look at the item/mob in-game, so translators think they can translate it literally.
41/10000+ is quite a positive result, because since the journey mode was added, it's just more visible and irritating to me, so I hope they'll fix it.

I guess people should be aware that context is needed to make good translations for what are often single words. Its still strange that this context was evidently not provided. This kinda sheds a bad light at the translators, even though its not their fault.

I remember when in the German version it was "Leertaste" for the Space zone. "Leertaste" is "space" but the key on the computer keyboard. It was corrected. I understand that translators are hard to get and in a rush to get the update in time if Loki/Redigit/Cenx says so.

If my memory serves me right, there used to be a similar bug in Polish, only the other way around. If you were to launch the on-screen keyboard while playing on a gamepad, the spacebar button was called "Kosmos".
