Nothing has scared me as of yet, but I'm sure the encountering of sprited spiders, my arachnophobia is that bad, left a little pee-pee in my Earthworm Jim suit. I'm not sure of anything else that I would class as 'scary' in this game.

I once encountered a boulder trap connected to an explosive trap, it didn't end well for me.

The spiders. First day of 1.2, I start mining after fanboying over all of the coolstuffs.
I get underground and then I see some type of monster in a cave. It goes away, then I mine into the cave.
INSTA REKT. I'M DEAD. ON THE FLOOR. HEART ATTACK. NO. STOP.

Late night + no contacts + dim lights + drowsiness = epic stupidity and blindness.

I also saw a Gold chest on the other side of that area, my eyes were affixed on that... unfortunatly.

dungeon guardian and the random explosives found underground. ( not the item, the trap that explodes when you hit the plate, but this is rare.)

most likely the explosive traps and dungeon guardian. also the lihzahrd traps sometimes scare me

Dungeon Guardian when he first meets you, traps from boulders and explosives(never ceases to surprise me), and anything but softmode/softcore
