The "Mecha Mayhem" achievement is earned by fighting and defeating 3 mechanical bosses at the same time. This is similar to the fight in Mechdusa, which also drops Waffle's Iron as a reward.
Waffle's Iron should also occur during normal gameplay, when fighting and defeating 3 mechanical bosses at the same time.
Since this is not the normal way to fight mechanical bosses, it would not be a common loot, in the same way as the Badger's Hat.
