PS4 Wall of Flesh Glitched?

In hell me and my friend were ambushed by a bunch of demons, i shot a voodoo demon down and the doll fell into the lava. The text saying the wall of flesh spawned showed up but me and him got insta killed by the wall who was nowhere near us and then it left?

Maybe there was a bit of a lag? Thats all that I can think of.

Could you better describe the circumstances? The manner of being "Insta-killed" as well as whether he was visible on-screen, on the map or nowhere at all changes the possible cause

Well, the moment he spawned we died and he was barely visible as in the tentacle hungries. My minimap was off so idk

Were you two on fire? Were there other enemies nearby?

How low into hell are you because once it spawns if you go up in depth to high and leave what is considered hell you instantly die if trying to escape happened to me on getfixedboi map. I just exploded said I tried to escape. I was on a minecart that dipped above the underworld by a block thats how mine happened so I was wondering your depth or death message.
