Every time you take damage, three boulders fall from the sky, but unlike stars they can harm you too. This could make for a hilarious gameplay challenge on Hardcore characters.

Should probably have two of the boulders fall with a bit of horizontal momentum from opposite top corners of the screen, so they can cross paths in the middle. Then have the third one just fall straight down in the middle.
