Not a gem cave, just a special background. There are all sorts of special cave backgrounds.

There are rare gem caves but not all of the special caves have gems.

A gem cave will always have a background that looks like stone with small gems embedded in it, and the gems in the background will match the gems in the cave. If the caves you found didn't have a background like that, then you were just getting lucky.

So, the world gen put a Blood Altar right next to the area I chose to build my house at the very center of the map:

It was almost perfectly aligned with the bottom of my house (I started on the house before I saw the altar). Normally I built the crafting room on the top but I decided this time we'd go for something different, so I put the planter area up on top and the crafting room down here (the opposite of how I normally do it). So now I can truly have every crafting station in my crafting room (if I don't run out of room lol).
