I'm not sure what you're asking, but Expert mode exclusive items have a unique rarity called Rainbow, which helps distinguish them from more common drops.

Yes, a little foggy on what you ask, but I do think what was said above should answer. The expert exclusive items (from treasure bags when you kill a boss) are rainbow rarity, and some of them don't work in normal worlds, such as the Demon Heart, where your extra slot will be inactive, and if an item is removed from it, the slot will be deleted. So that's something to be cautious about if you use sub-worlds.
