Make sure there are no projectiles in the air from your weapon when you drop the doll. See if that makes a difference.

There never was when I did that.

What weapons are you using for the fight?

Beenade, Bee Gun, and Night's Edge

Make sure there are absolutely no bees in the air, even off screen, when you put the doll in lava. Try starting a new session, go down to Underworld, put doll in without using any weapons at all to test if it works.

It's now water bolt only and there was no projectiles in the air when the guide was killed
