Chaos Elementals only spawn on pearlstone, pearlsand, and pink ice when underground, and not in front of a wall

Unless the box is your arena. Probably with Chlorophyte arrows against plantera.
Buffling
If you reeeeeealy needed to, you could just put the nurse inside of a house and use solar eruption and heal whenever you take damage. Then of course add campfires and stand in honey to make it easier.
