Banners sometimes not dropping when they should

Sometimes, whenever I kill 50 of an enemy, no banner spawns. In fact, there isn't even a message in chat saying that the 50th enemy was killed. It has happened to me three times, one with Moths, one with Zombie Elves, and one with Martian Drones. All three times, it occurred while farming enemies using a volcano trap, utilizing lava. The most recent one, the Drone, there were no other traps involved, and I can't remember about the others.
 
Did you have a Tally Counter available to see the kills? Some methods of killing enemies, such as lava/traps do not count as a player kill, and so will not count towards banner progress.
 
After doing some thinking, I think the Zombie Elf one did not actually have lava involved. My thought before is that there was a bug that would cause lava to occasionally count towards banner progress, and whenever that occured on the 50th kill, no banner was obtained since no player killed it, although that kind of disproves it.
