To re-create:
Bind a hotbar slot to a random key you don't use (for example, slot 7 to P)
Press the key you bound it to. It should be selected.
Use an item (ideally something slow like a mushroom spear) and while it's swinging press that key again. It doesn't do anything.
Then, use the item again, and press the default key for the slot instead of your custom hotkey. It selects the slot.

EDIT: If you're still holding the custom key at the end of the items use time, it does select the slot correctly. Tried this after reading this post.
This makes it difficult to buffer swaps with custom hotkeys.
I noticed in the input profiles.json file there are 2 stored control schemes aside from your custom ones, named "Redigit's Pick" and "Yoraiz0r's Pick." Maybe it has something to do with those?
I don't know if it happens on other platforms.
