PC Wall of Flesh glitched

Hey guys, apologies if this is in the wrong section, but I think my WoF is glitched. Whenever I throw in a guide doll, the Guide is slain but the wall of flesh isn't spawned in. Yes, the Guide was alive and I did keep the doll on screen when it went in the lava, so I don't know what's wrong.

Also... a dumb question: did you throw it in the Underworld/Hell? It only does spawn there.
