Found this in the change log. It only happens in expert mode. Finally a challenge worth the hype!

I looked at the first 6, the rest I'd rather not know about. I just want to play through the game and see what I get an achievement for doing. But that might just be me.

It's one regarding a sword, Supposedly made out of 4 lesser swords and then refined into an even greater item.
