A rare pet item found in Skyware chests on Floating Islands. The item goes in the pet slot and spawns a small angel that follows you and rests around your shoulder. The shoulder angel spawned has short blonde hair, blue eyes, and a halo. He wears a white robe. The item sprite is just a small little golden angel medal, and has Pink rarity.
The Shoulder Angel seems to write in a scroll as an animation, like it is recording your actions. Its facial expression shows you, as a comedic gag, if what is written is considered a good or bad deed. For example, if you kill an NPC, the Shoulder Angel looks angry and writes red lines. however, if you kill a boss or house an NPC, it smiles and writes green text. None of this affects gameplay.
If you die in Hardcore, the book is finished, and the Shoulder Angel flies upward with it. Like you were judged or something. If you die in another difficulty, the angel mourns and “prays” for you to come back.
The Shoulder Angel looks fearful in the Underworld, and cowers when near demons or other monsters. It seems at peace in the high sky, relaxing sometimes.

Tooltip vaguely references the Hazbin Hotel universe being musical. XD
