If A zombie in real life (Movies) can infect
how about zombies can infect NPCs
*Stages of infection*
0%: NPC isn't infected and okay for now
5%: They are not much green so they are okay for now...
20%: a bit more infected so they okay but I think you should cure them
50% NPC is half zombie, cure them
78% Not long left!
100% NPC will infect other NPCs and will be fully green
How to cure them: Have the nurse heal them with the syringe, Cure them with a special potion, If they respawn they will be at 0%
 
NPCs are as weak as wet paper and if they're getting injured by zombies, they'll probably be dead in a few seconds anyway. (Especially if you're playing Expert mode.) Once you've got a decent sword, zombies are also about as threatening as wet paper.
Why should we cure infected NPCs? What does an infected NPC do? What about non-zombie enemies that can also threaten NPCs? (Flying eyes, goblins, pirates, Martians, Solar Eclipse horrors.)
 
once an NPC is infected would other NPC's attack them? And if the infected NPC is on a different floor how would the nurse heal them? And how would you acquire the potion?

First: Nope and would only flee
Second: The Nurse Would walk down the stairs to them or throw the syringe down the platform trying to hit the infected one
Third: 13% Chance of it being dropped by any zombie
 
This would only be tedious for players and programmers respectively. You get a potion that you can only use on NPCs (And given that this would be from early game on, if it were made to be a weapon, it wouldn't have much damage.) They get infected? Not only do you have to wait, assuming you don't get a potion, if you're away from your main base or the NPCs for extended periods of time, the next thing you know, everyone's been infected, or worse, killed, and you'll have to either waste time farming the potions so you don't have to deal with dying NPCs or let them die and hope they respawn quickly enough for you to do your business.
That's not even going into the logistics of what it would take to implement this. The NPC pathfinding isn't the best, they'll fall into lava if you're not careful, not only would this require a massive overhaul just so the nurse can do her job (Which if she does it ASAP, means the potion will essentially be useless, and don't forget she can heal herself).
Essentially, it's a lot of setup for a system that just won't work, there's no danger to it, at worst it'll make blood moons a little worse, but placing a banner on a wall prevents doors from opening so there's no issue there. The concept is fine, the idea is fine, and perhaps with tweaks done it can be a serviceable suggestion, but presently, it won't work out.
 
See, the problem is, I don't see why do you want this to be a thing. Does it improve the game in any way? Or make it more fun? Or you made this because you just think it's supposed to be that way because zombies can infect human in other games/films?

I don't see the point of this. Not like you put in many details on what an infected NPC would do anyway.
