The most evil trap ideas you can think of.

A "Trick Boulder" trap, which randomly replaces a boulder trap. The boulder it spawns with does not contact the player and does very little damage, intended only to scare or surprise them. This is merely an illusion of safety, however, as after a short period, if the player is stationary for over a half-second, an explosive boulder drops and detonates on contact, instantly killing the player even at max health and defence. It can be avoided if you're quick, but it does destroy blocks.

The "Ominous Fumes" item, which randomly spawns in chests deep underground. Once opened, it gets consumed, activating a long-lasting debuff. Enemy spawn rates slowly increase globally, starting out very minor but gradually rising until you can consider yourself effectively dead. Enemies that normally spawn exclusively in other biomes may spawn anyway (for example Jungle enemies in the Tundra). The effect expires after the player achieves 100 thousand enemy kills. It does not expire on death or if the player leaves the world. On Expert and above, random bosses may also spawn.
 
A trap that has a 1/2 chance of generating on any given world.
It's just wire and actuators covering the whole world, so if you activate any pressure plate, switch, detonator, etc, the whole world actuates.
That would crash the game. Unless you mean o my the original boulder duplicates. Then it would be ok. But if it was every boulder, the exponential increase in entities would quickly overload the game’s memory.
Actually, there is a projectile limit of 1000.
 
What about an early-game enemy that duplicates itself? But the catch... is that the more there are, the higher stats they have.

the dead-man switch: when wired to something such as an actuator or trap, then said wired device will activate if cut off from said switch in any way, even by cutting wires or breaking the switch itself

trapped (black) pressure plate: when stepped on, the pressure plate is "primed", and stepping off of the pressure plate, or severing the connection to the plate, will activate every wired device it is connected to
 
Medusa Shrine

Small structure with a chest in the middle and two pedestals next to it. Both have what looks like a cloth over something. Under one is a rock, but under the other is a severed Medusa head. When the chest is opened, the cloths come off. If the player is facing the rock, nothing happens. But if they’re facing the Medusa head, they are inflicted with the Stoned debuff, and the blocks below them (between twenty-five and fifty) are actuated, causing them to fall.
The chest loot has a chance to have a special golden sword which I will edit for info.

This can already be achieved through a T-flip-flop and a weighted pressure plate:

The trap will only activate once every two pulses, so with this setup it will only activate on the second tick (that of the player stepping off)

This will not activate if the wire is cut, which is actually pretty much impossible to detect because wiring doesn't hold state.



The most evil trap I can think of would be a pack of explosives hooked up to a pressure plate that will just blow up immediately for no reason...
Oh, wait. That already exists.
