It only works up to the most powerful pick in your inventory. No pick, no block swapping.

but am I the only one who finds this rather immersion breaking?

Like... how is that justified in-game?

I think this would actually bother me a lot... can we get a toggle for this in settings? Please?

P.S.: Have you ever fixed that one weapon you couldn't reforge at all... the bugged one?
