Currently in Terraria you can change your appearance in most ways. The hair stylist can change your hair style and color. You can change your clothes at a dresser. And you can change your name by altering game files( I think). The only thing you can't change is skin color. And eye color. There's that too. What I'm saying is there could be a new npc called the plastic surgeon. He would change skin and eye color for a somewhat high price. The cost would depend on how different the new color was. For example, changing red to orange would cost less that changing red to purple. This applies to both eyes and skin.
I currently have no idea how you would get him. If you have ideas, please tell me.
